ABSTRACT

The underground environment is dark and humid, and it is easy to breed pathogenic microorganisms. A lump in the right lung of a coal mine underground transport worker was found druing occupational health examination. CT examination showed that the lump was located in the posterior segment of the upper lobe of the right lung, with point strip calcification, liquefaction necrosis, and proximal bronchial stenosis and occlusion. MRI examination FS-T(2)WI and DWI showed "target sign", annular low signal around the central high signal, and low mixed signal around the periphery, and annular high signal in the isosignal lesions on T(1)WI. Then the pulmonary aspergillus infection was confirmed by pathology.

ABSTRACT

Background: To control the spread of coronavirus disease-19 (COVID-19) caused by the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), it is necessary to adequately identify and isolate infectious patients particularly at the work place. Real time polymerase chain reaction (RT-PCR) assay is the recommended confirmatory method for the diagnosis of SARS-CoV-2 infection. The aim of this study was to determine the prevalence of SARSCoV-2 infection in Burkina Faso and to use the initial cycle threshold (Ct) values of RT-PCR as a tool to monitor the dynamics of the viral load. Methodology: Between September 2021 and February 2022, oropharyngeal and/or nasopharyngeal swab samples of consecutively selected COVID-19 symptomatic and apparently healthy workers from the Wahgnion mining site in the South-western Burkina Faso who consented to the study were collected according to the two weeks shift program and tested for SARS-CoV-2 using RT-PCR assay. Patients positive for the virus were followed-up weekly until tests were negative. Association of the initial RT-PCR Ct values with disease duration was assessed by adjusted linear regression approach. Two-sided p value < 0.05 was considered statistically significant. Results: A total of 1506 (92.9% males) participants were recruited into the study, with mean age and age range of 37.18.7 and 18-68 years respectively. The overall prevalence of SARS-CoV-2 infection was 14.3% (216/1506). Of the 82 patients included in the follow-up study, the longest duration of positive RT-PCR test, from the first positive to the first of the two negative RT-PCR tests, was 33 days (mean 11.6 days, median 10 days, interquartile range 8- 14 days). The initial Ct values significantly correlated with the duration of RT-PCR positivity (with ß=-0.54, standard error=0.09 for N gene, and ß=-0.44, standard error=0.09 for ORF1ab gene, p<0.001). Participants with higher Ct values corresponding to lower viral loads had shorter viral clearance time than those of lower Ct values or higher viral loads. Conclusion: Approximately 1 out of 7 tested miners had SARS-CoV-2 infection and the duration of their RT-PCR tests positivity independently correlated with the initial viral load measured by initial Ct values. As participants with lower initial Ct values tended to have longer disease duration, initial RT-PCR Ct values could be used to guide COVID-19 patient quarantine duration particularly at the work place.


Contexte: Pour contrôler la propagation de la maladie à coronavirus 19 (COVID-19) causée par le syndrome respiratoire aigu sévère coronavirus-2 (SRAS-CoV-2), il est nécessaire d'identifier et d'isoler de manière adéquate les patients infectieux, en particulier sur le lieu de travail. Le test de réaction en chaîne par polymérase en temps réel (RT-PCR) est la méthode de confirmation recommandée pour le diagnostic de l'infection par le SRAS-CoV-2. Le but de cette étude était de déterminer la prévalence de l'infection par le SRAS-CoV-2 au Burkina Faso et d'utiliser les valeurs du seuil initial du cycle (Ct) de la RT-PCR comme outil de suivi de la dynamique de la charge virale. Méthodologie: Entre septembre 2021 et février 2022, des écouvillonnages oropharyngés et/ou nasopharyngés de travailleurs symptomatiques COVID-19 et apparemment en bonne santé sélectionnés consécutivement du site minier de Wahgnion dans le sud-ouest du Burkina Faso qui ont consenti à l'étude ont été prélevés selon les deux programme de quart de semaines et testé pour le SRAS-CoV-2 à l'aide d'un test RT-PCR. Les patients positifs pour le virus ont été suivis chaque semaine jusqu'à ce que les tests soient négatifs. L'association des valeurs Ct initiales de la RT-PCR avec la durée de la maladie a été évaluée par une approche de régression linéaire ajustée. Une valeur p bilatérale < 0,05 a été considérée comme statistiquement significative. Résultats: Un total de 1506 participants (92,9% d'hommes) ont été recrutés dans l'étude, avec un âge moyen et une tranche d'âge de 37,1 à 8,7 ans et de 18 à 68 ans, respectivement. La prévalence globale de l'infection par le SRAS-CoV-2 était de 14,3% (216/1506). Sur les 82 patients inclus dans l'étude de suivi, la plus longue durée de test RT-PCR positif, du premier test positif au premier des deux tests RT-PCR négatifs, était de 33 jours (moyenne 11,6 jours, médiane 10 jours, intervalle interquartile 8-14 jours). Les valeurs Ct initiales étaient significativement corrélées à la durée de positivité de la RT-PCR (avec ß=-0,54, erreur standard=0,09 pour le gène N et ß=-0,44, erreur standard=0,09 pour le gène ORF1ab, p<0,001). Les participants avec des valeurs de Ct plus élevées correspondant à des charges virales plus faibles avaient un temps de clairance virale plus court que ceux avec des valeurs de Ct plus basses ou des charges virales plus élevées. Conclusion: Environ 1 mineur testé sur 7 était infecté par le SRAS-CoV-2 et la durée de la positivité de ses tests RTPCR était indépendamment corrélée à la charge virale initiale mesurée par les valeurs Ct initiales. Comme les participants avec des valeurs Ct initiales inférieures avaient tendance à avoir une durée de maladie plus longue, les valeurs Ct initiales de la RT-PCR pourraient être utilisées pour guider la durée de la quarantaine des patients COVID19, en particulier sur le lieu de travail.

Se analiza el comportamiento de los accidentes de trabajo mortales (ATM) y de las enfermedades profesionales (EP), en relación con las empresas del régimen general en la minería peruana, durante los años 2010 al 2019. Se realizó un estudio transversal de la base de datos del Ministerio de Energía y Minas. El estudio incluyó las notificaciones de accidentes de trabajo mortales y de las enfermedades profesionales reportadas en esa década por las empresas mineras del régimen general; del 2010 al 2019 para los ATM, y entre 2011- 2019 para las EP, a nivel nacional. Resultados: De 422 ATM notificados entre 2010-2019 se determinan cuatro rangos, considerando 126 empresas. El rango D (dos empresas) son el 2% pero las que presentan la mayor siniestralidad laboral. La empresa D1 acredita 29 ATM y la empresa D2 tiene19 ATM, ambas asumen el 11.34% del total de los ATM en la década analizada. El rango A, son 95 empresas, conforman el 75% del total y presentan entre 1 a 3 ATM para el mismo periodo. El 61% de trabajadores víctimas de ATM son de contratistas y conexas; mientras que el 39% son ATM de las empresas principales. Entre 2011-2019 se notificaron 40,001 EP en las empresas mineras, trabajadores con menos de un año registran EP y mayores de 55 años tienen una curva pronunciada de EP. El 1% son trabajadoras mineras y el 99% son trabajadores. Se necesita mayor investigación sobre las EP en el sector minero(AU)


To analyse the behavior of fatal work accidents (ATMs for its acronym in Spanish) and occupational diseases (EPs), in companies under the general regime of the mining sector in Peru during the years 2010 to 2019. A cross-sectional study was carried out using the database from the Ministry of Energy and Mines (MINEM). The study included notifications at the national level of ATMs (2010-2019) and EPs (2011 to 2019) reported by the mining companies under the general regime. Results: Of 422 ATMs reported by 126 companies between 2010-2019, four range categories were determined. Rank D grouped two companies (representing only 2%) that had the highest occupational accident rate. Company D1 reported 29 ATMs and company D2 reported 19 ATMs, together accounting for 11.34% of the total number of ATMs during the study period. Rank A consisted of 95 companies (representing 75%) and reporting between 1 to 3 ATMs for the same period. Contractor and outsourcing companies reported 61% of workers victims of ATMs, while main companies reported 39% of ATMs. Between 2011-2019, 40,001 EPs were reported in mining companies, workers under one year reported EPs, and workers over 55 years of age had a steep EP curve. Female workers represent 1% of mining workers and 99% are male workers. More research is needed on EPs in the mining sector(AU)

Abstract Background: Due to the growing concern about work-related social and health aspects, occupational health and safety has become relevant. Objective: This work aims to develop a model to assist cardiovascular risk management in a team of haul truck operators, who work in rotating shifts at a mining company in Brazil. Methods: This longitudinal study evaluated risk factors for cardiovascular diseases of 191 mineworkers at three times points - 2010, 2012, and 2015. In addition, the risk of developing cardiovascular diseases was calculated, and the risk factors were analyzed using the chi-square test, the U Mann-Whitney test, and binary logistic regression. The significance level was set at 5%. Results: In the study period, body weight, body mass index (BMI), waist-to-height ratio (WHR), systolic (SBP), and diastolic blood pressure (DBP), total cholesterol, high-density lipoprotein cholesterol (HDL-C), and triglycerides levels of the study group increased. In 2015, there was a high prevalence of alcohol intake, overweight or obesity, central obesity, inadequate WHR, high blood pressure, total cholesterol above 190 mg/dL, and triglycerides above 150 mg/dL. An association was identified between increased cardiovascular risk and age, SBP, HDL-C, low-density lipoprotein cholesterol (LDL-C), and elevated glucose levels. Conclusion: Intense interventions for reduction and prevention of elevated alcohol intake, blood pressure levels, WHR, metabolic syndrome, blood glucose, and LDL-C levels, and low HDL-C levels are needed. In addition, a close monitoring of mine workers over 38 years of age who smoke, consume alcoholic beverages, and have altered blood glucose levels is important.

ABSTRACT

Background: Operator drivers are responsible for driving trackless heavy-duty mining or construction vehicles such as graders, dumpers, loaders, and bulldozers. They have an increased prevalence of musculoskeletal disorders (MSDs) due to the nature of their work. There is a paucity of data on MSDs and associated factors among operator drivers in Namibia. Objective: We assessed awareness of, and factors associated with, MSDs among operator drivers in the construction and mining industries in Namibia. Methods: In this cross-sectional study, questionnaires were administered to operator drivers, and managerial staff were interviewed, using a semi-structured interview guide. Data obtained were analysed using chi-square tests and binary logistic regression modelling. Results: 182 operator drivers completed the questionnaires, and 13 operator drivers' supervisors and managers were interviewed. Factors associated with MSDs were length of service for ≥ 10 years (OR 15.3, 95% CI 6.0­39.0), alcohol consumption (OR 2.8, 95% CI 1.1­6.7), lack of physical fitness activity (OR 8.8, 95% CI 3.8­20.4), and lack of awareness of MSDs (OR 3.1, 95% CI 1.3­7.3). Managerial staff were of the opinion that the operator drivers did not suffer from MSDs. Conclusion: We found health issues that are associated with MSDs among the operator drivers, impacting their general wellness and productivity. Monitoring of health and wellness of these workers by the companies is recommended. There is need to increase MSD awareness, personal protective equipment usage, ergonomics skills training, and physical fitness exercises for operator drivers. Employers are urged to adopt policies, and to design guidelines and interventions aimed at promoting occupational health and safety in this population.

Objective: To detect of gene expression and genotype of the ataxia telangiectasia mutated (ATM) from coal workers' pneumoconiosis (CWP) , It is explored whether CWP is related to ATM gene. Methods: In October 2020, the relevant information of 264 subjects who received physical examination or medical treatment in the Department of occupational diseases of Guiyang public health treatment center from January 2019 to September 2020 was collected. Through the occupational health examination, 67 healthy people with no history of exposure to occupational hazards were selected as the healthy control group; The coal miners with more than 10 years of coal dust exposure history and small shadow in the lung but not up to the diagnostic criteria were the dust exposure control group, a total of 66 people; The patients with the same history of coal dust exposure and confirmed stage I were coal worker's pneumoconiosis stage I group, a total of 131 people. The expression of ATM was detected by QRT PCR. ATM rs189037 and rs1801516 were genotyped by massarray. Results: There was significant difference in the expression of ATM among the groups (P<0.05) ; Compared with the healthy control group, the expression of ATM in the dust exposed control group was significantly increased (P<0.05) . With the occurrence and development of CWP, the GG of rs189037 wild type decreased, the GA of mutant heterozygote and AA of homozygote increased, but the difference was not statistically significant (P>0.05) ; Rs1801516 wild type GG and mutant heterozygote GA had no significant changes (P>0.05) . There were significant differences in age, neutrophils and basophils among rs189037 groups (all P<0.05) . There were no significant differences in blood pressure, eosinophils, lymphocytes, monocytes, smoking and drinking history among rs189037 groups (all P>0.05) . Compared with wild-type GG, the or of mutant heterozygotes and homozygotes increased, but the differences were not statistically significant (P>0.05) . Conclusion: ATM gene may be one of the early activation genes of CWP and rs189037 may be the functional loci which affects gene expression. ATM gene is related to inflammatory response, Neutrophils and basophils have an impact on the development of CWP.

ABSTRACT

Este artigo visa a apresentar uma pesquisa-intervenção voltada para a compreensão da relação entre família e trabalho, em uma unidade mineradora situada no Norte do Brasil. Adotam-se aqui olhares críticos da psicologia sobre o trabalho. Embora não estivessem previstos originalmente, os dados aqui relatados emergiram do campo, derivados do projeto de pesquisa-intervenção contratado pela empresa, que buscava investigar as variáveis psicossociais que diretamente afetam o trabalho de Operadores de retroescavadeira. A necessidade de buscar maior aproximação dos sujeitos de pesquisa motivou a realização de uma intervenção junto às suas famílias. Essa intervenção foi feita a partir de técnicas grupais, como rodas de conversa e visitas domiciliares, seguida pela análise das percepções dos familiares sobre o trabalho dos Operadores e sobre a relação desses com a empresa. Os resultados destacam a importância do conceito de território para compreender os processos de subjetivação das famílias; os vínculos sociais como fator psicossocial protetivo; a imbricada articulação entre o processo de gestão do trabalho e a política de benefícios sociais. Concluímos destacando a importância dos vínculos familiares para a compreensão real da atividade dos Operadores, e reafirmando a noção de centralidade do trabalho, na conformação do território.


This article aims to present an intervention research focused on understanding the relationship between family and work in a mining unit located in Northern Brazil. Psychology's critical views on work are adopted here. Although not originally foreseen, the data reported here emerged from the field, derived from the intervention research project contracted by the company, which sought to investigate the psychosocial variables that directly affect the work of backhoe operators. The need to get closer to the research subjects motivated the realization of an intervention with their families. This intervention was carried out using group techniques, such as conversation circles and home visits, followed by an analysis of family members' perceptions about the operators' work and their relationship with the company. The results highlight the importance of the concept of territory to understand the subjectivation processes of families; social bonds as a protective psychosocial factor; the imbricated articulation between the work management process and the social benefits policy. We conclude by highlighting the importance of family ties for the real understanding of the operators' activity, and reaffirming the notion of the centrality of work in the shaping of the territory.

RESUMEN La responsabilidad social, como una de las respuestas a los actuales conflictos socioambientales, implica esfuerzos adicionales de las organizaciones para fortalecer su capacidad de gestión. Esto sucede, de manera especial, en las compañías mineras, cuyas actividades comprometen el uso de recursos de comunidades, entre ellos los naturales. Esta investigación analizó los aportes que la gestión de comunicaciones hace al relacionamiento con las comunidades en el marco de dos programas de responsabilidad social de los proyectos mineros Quebradona y Gramalote (ubicados en Antioquia, Colombia), adscritos a la compañía Anglogold Ashanti y ganadores del Premio Sello Social a la Minería en Antioquia, siendo dos de los proyectos con mayor exigencia y veeduría ciudadana en la región. Desde una mirada con predominancia interpretativa, y acudiendo a la revisión de documentos y entrevistas a jefes de áreas sociales, comunicadores, beneficiarios y líderes ambientales, se concluyó que el análisis del entorno, la visibilización de los programas y la mediación en los conflictos socioambientales son los principales aportes de los comunicadores en los proyectos mineros.


ABSTRACT Social responsibility, as one of the responses to current sock-environmental conflicts, implies additional efforts by organizations to strengthen their management capacity. This happens, especially, in mining companies, our activities compromise the use of community resources, including natural resources. This research analyzed the contributions that communications management makes to relations with communities within the framework of two social responsibility programs of the Quebradona and Gramalote mining projects (located in Antioquia, Colombia), attached to the company Anglogold Ashanti and winners of the Premio Sello Social a la Minería en Antioquia, being two of the projects with the highest demand and citizen oversight in the region. From an interpretive perspective and going to the review of documents and interviews with heads of social areas, communicators, beneficiaries, and environmental leaders, it was concluded that the analysis of the environment, the visibility of the programs and mediation in socio-environmental conflicts are the main contributions of communicators in mining projects.

ABSTRACT

High altitude (HA) mining operations are a very important business in Chile, but reduced availability of oxygen affects the sleep quality, increasing the risk of accidents. An important regulator of sleep-wake cycle is the hormone Melatonin, produced by pineal gland as a sleep inductor. The aim of this study is to evaluate the effect of high altitude (4,500 m) on the quality of sleep of workers undergoing to Chronic Intermittent Hypobaric Hypoxia (CIHH) using self-reported surveys of sleepiness and sleep quality, measurement of sleep apnea (using nocturnal oximetry) and serum levels of melatonin. The Desaturation index (ID4) results revealed higher HA scores compared to sea level (SL). Regarding melatonin levels, the results show that it is increased in HA versus SL and this increase would be related to oxygen saturation during sleep. These data link sleep quality in HA to its melatonin levels, suggesting that melatonin may be a potential biomarker for sleep quality.

Abstract Background A coalmine disaster has occurred in Turkey on 13 May 2014, which caused the death of 301 miners. Objective This study aimed to determine the prevalence of posttraumatic stress disorder (PTSD) symptoms and the risk factors for PTSD among mine workers, 2 years after the coalmining disaster. Methods This was a cross-sectional survey conducted between June 2016 and July 2016 among mine workers who were the employees of either the mine where the accident occurred or three other mines in the same area. Sociodemographic data form and PTSD Symptom-Scale Self-Report (PSS-SR) were used to collect data. Results 672 mine workers participated in the study. At the time of the accident, 23.7% (n = 159) of them were in the mine where the accident occurred. The mean score on total PSS-SR was 4.27 (SD: ±4.49). Eighteen (2.7%) participants screened positive for PTSD. Logistic regression analysis revealed the significant risk factors for PTSD are: being single/divorced/widowed, having a chronic disease, having a family history of psychiatric illness and previously experiencing traumatic events more than one. Discussion Coalmining workers have considerably high prevalence rate of PTSD symptoms after a coalmining disaster. Assessing PTSD and associated risks is important for preventive mental health services.

BACKGROUND The number of malaria cases in Roraima nearly tripled from 2016 to 2018. The capital, Boa Vista, considered a low-risk area for malaria transmission, reported an increasing number of autochthonous and imported cases. OBJECTIVES This study describes a spatial analysis on malaria cases in an urban region of Boa Vista, which sought to identify the autochthonous and imported cases and associated them with Anopheles habitats and the potential risk of local transmission. METHODS In a cross-sectional study at the Polyclinic Cosme e Silva, 520 individuals were interviewed and diagnosed with malaria by microscopic examination. Using a global positional system, the locations of malaria cases by type and origin and the breeding sites of anopheline vectors were mapped and the risk of malaria transmission was evaluated by spatial point pattern analysis. FINDINGS Malaria was detected in 57.5% of the individuals and there was a disproportionate number of imported cases (90.6%) linked to Brazilian coming from gold mining sites in Venezuela and Guyana. MAIN CONCLUSIONS The increase in imported malaria cases circulating in the west region of Boa Vista, where there are positive breeding sites for the main vectors, may represent a potential condition for increased autochthonous malaria transmission in this space.

ABSTRACT

Background: Current initiatives in southern Africa to medically assess former migrant miners for silicosis and tuberculosis, including statutory and lawsuit derived compensation programmes, require burden of disease information. nObjective: To use clinical information collected on ex-miners examined at the Occupational Health Service Centre (OHSC) in Lesotho, operated under the Tuberculosis in Mining Sector in Southern Africa (TIMS) project, to measure the burden of lung disease and respiratory impairment. Methods: Demographic, occupational and medical history information, chest radiology, spirometry, GeneXpert testing for tuberculosis, and pulse oximetry outcomes were analysed, and descriptive summary measures calculated, in a group of ex-miners examined in 2017 and 2018. Results: The study sample comprised 2 758 Basotho former underground miners, with median age of 62 years and median length of service of 28 years. Among ex-gold miners (n = 2 678), disease prevalence was high: radiological tuberculosis (consistent with previous or current disease) 60.9%, silicosis 42.5%, HIV 30.7%, silicotuberculosis 25.7%, and current active tuberculosis 6.8%. Of those with tuberculosis diagnosed microbiologically, 6.7% had no radiological evidence of tuberculosis and 54.1% did not report cough. Conclusion: The findings have public health and compensation implications. There are large numbers of ex-miners with potentially compensable disease under both the statutory system and a settlement trust set up following litigation. This overlaps with a tuberculosis-HIV co-epidemic which requires screening and treatment for tuberculosis and HIV, and managing a considerable disability and care burden on families and the Lesotho health system. Coordinated planning and substantial resources are needed for these programmes to do justice to their mandates

ABSTRACT

Background: Occupational noise-induced hearing loss (ONIHL) is one of the most common occupational health diseases affecting miners in South Africa. Accurate and appropriate medical data are essential for making valid diagnoses. Objectives: The purpose of this study was to describe the electronic records of a South African platinum mine's audiometry medical surveillance system and their role in early diagnosis of ONIHL. Ear-related conditions of affected miners, occupations, and noise levels were concurrently reviewed, and the characteristics of miners with and without ONIHL were described. Methods: This was an analysis of secondary data from the electronic audiometry and employee occupational records of 305 platinum mine workers for the period 2014 to 2017. Data were analysed using descriptive statistics. Results: Although the audiometry records contained appropriate and relevant data, including annual hearing screening percentage loss of hearing (PLH) shifts, there was evidence of inaccurate and insufficient recording of risk factors for hearing loss in the medical surveillance records. The records indicated that the miners in some occupations were exposed to dangerously high noise levels, exceeding 85 dB(A). Miners as young as 21 years of age were diagnosed with ONIHL. Conclusion: The insufficient and inaccurate data captured in the miners' records has important implications for the mine's efficient implementation of hearing conservation programme (HCP) elements aimed at mitigating ONIHL. The hazardous noise levels recorded call for increased attempts to meet noise level regulations, while the presence of conditions such as pseudohypacusis highlights the need for exploration of more reliable assessment measures

ABSTRACT

OBJECTIVES: This study investigated the indirect effect of calcium-enriched mixture (CEM) cement and mineral trioxide aggregate (MTA), as 2 calcium silicate-based hydraulic cements, on human dental pulp stem cells (hDPSCs) through different dentin thicknesses.MATERIALS AND METHODS: Two-chamber setups were designed to simulate indirect pulp capping (IPC). Human molars were sectioned to obtain 0.1-, 0.3-, and 0.5-mm-thick dentin discs, which were placed between the 2 chambers to simulate an IPC procedure. Then, MTA and CEM were applied on one side of the discs, while hDPSCs were cultured on the other side. After 2 weeks of incubation, the cells were removed, and cell proliferation, morphology, and attachment to the discs were evaluated under scanning electron microscopy (SEM). Energy-dispersive X-ray (EDXA) spectroscopy was performed for elemental analysis. Alkaline phosphatase (ALP) activity was assessed quantitatively. The data were analyzed using the Kruskal-Wallis and Mann-Whitney tests.RESULTS: SEM micrographs revealed elongated cells, collagen fibers, and calcified nucleations in all samples. EDXA verified that the calcified nucleations consisted of calcium phosphate. The largest calcifications were seen in the 0.1-mm-thick dentin subgroups. There was no significant difference in ALP activity across the CEM subgroups; however, ALP activity was significantly lower in the 0.1-mm-thick dentin subgroup than in the other MTA subgroups (p < 0.05).CONCLUSIONS: The employed capping biomaterials exerted biological activity on hDPSCs, as shown by cell proliferation, morphology, and attachment and calcific precipitations, through 0.1- to 0.5-mm-thick layers of dentin. In IPC, the bioactivity of these endodontic biomaterials is probably beneficial.

ABSTRACT

Chronic Kidney disease (CKD) disturbs mineral homeostasis leading to mineral and bone disorders (MBD). Serum calcium and phosphate (Pi) remain normal until the late stages of CKD at the expense of elevate fibroblast growth factor-23 (FGF-23), a phosphaturic hormone, followed by reduced 1,25-dihydroxy-vitamin D (1,25[OH]2D) and finally elevated parathyroid hormone (PTH). Pi retention is thought to be the initial cause of CKD-MBD. The management of MBD is a huge clinical challenge because the effectiveness of current therapeutic regimens to prevent and treat MBD is limited. An intermittent regimen of PTH, when administered at the early stages of CKD, through its phosphaturic action, could prevent FGF-23 increases, the drop of 1,25(OH)2D, and the development of renal osteodystrophy, including secondary hyperparathyroidism (HPT) and its catabolic effects on the skeleton. Even in more advanced stages of CKD that have not progressed to tertiary HPT, could be beneficial. Therapeutic effects could be achieved in vascular calcification as well. Limited experimental/clinical data support the effectiveness of PTH in CKD-MBD. Its safety, has been established only when it is used for the treatment of osteoporosis, including patients with CKD. The proposed intermittent PTH administration is biologically plausible but its effectiveness and safety has to be critically assessed in long term prospective studies in patients with CKD-MBD.

ABSTRACT

BACKGROUND: Adequate suppression of bone turnover rate is important to decrease fracture risk without mineralization defect due to oversuppression. This study was performed to determine reference intervals (RIs) for 2 bone turnover markers, serum C-terminal telopeptide of type I collagen (CTX) and osteocalcin, in Korean women.METHODS: A total of 461 Korean women (287 premenopausal and 174 postmenopausal) without any disease or drug history affecting bone metabolism was included. Serum CTX and osteocalcin were measured after overnight fasting. Bone mineral density (BMD) was measured at the 1st to 4th lumbar vertebra using dual energy X-ray absorptiometry. Subjects with normal spinal BMD (T-score ≥−1.0) were included in this study.RESULTS: After stable concentrations were maintained, both CTX and osteocalcin were abruptly increased in 50 to 59 years, and then decreased with increasing age. Median levels and interquartile range of serum CTX and osteocalcin in all subjects were 0.322 (0.212–0.461) ng/mL and 15.68 (11.38–19.91) ng/mL. RIs for serum CTX and osteocalcin in all subjects were 0.115 to 0.861 ng/mL and 6.46 to 36.76 ng/mL. Those were higher in postmenopausal women (CTX, 0.124–1.020 ng/mL, osteocalcin, 5.42–41.57 ng/mL) than in premenopausal women (CTX, 0.101–0.632 ng/mL, osteocalcin, 6.73–24.27 ng/mL). If we use target reference levels as lower half of premenopausal 30 to 45 years in patients with antiresorptive drugs, those were 0.101 to 0.251 ng/mL and 6.40 to 13.36 ng/mL.CONCLUSIONS: We established RIs for serum CTX and osteocalcin in healthy Korean women with normal lumbar spine BMD. Premenopausal RIs for serum CTX and osteocalcin would be useful to monitor patients with low bone mass using osteoporosis drugs.

RESUMEN Objetivo Determinar el impacto percibido en la salud de los mineros artesanales del municipio de Quinchía (Colombia) por el uso de mercurio y cianuro en el proceso de amalgamiento de oro. Métodos Investigación cuantitativa, descriptiva, en una muestra de 28 mineros activos expuestos a cianuro y mercurio. Se caracterizó el proceso de amalgamiento de oro, se realizó una encuesta de percepción de síntomas y se efectuaron exámenes de laboratorio para detectar la concentración de mercurio y cianuro en la sangre. Resultados El 50% de los mineros llevan en el oficio entre 21 y 30 años, con una exposición de 2,6 horas/día al mercurio y cianuro. El 33% de los mineros presentan niveles de concentración de mercurio en la sangre entre 4 y 4,9 microgramos por litro (μg/L) y refieren síntomas en los sistemas nervioso y digestivo. Los resultados de cianuro en la sangre fueron negativos. Conclusiones Las concentraciones de mercurio en la sangre se asocian con el tiempo en el oficio, horas de exposición al mercurio y percepción de síntomas. Los resultados de cianuro en la sangre no son coincidentes con los síntomas referidos por los mineros. Pese a que, se reconoce la toxicidad de estas sustancias químicas, aún se emplean métodos tradicionales y prohibidos sin tener en cuenta los protocolos de seguridad y el uso de elementos de protección adecuados. Los mineros constantemente están expuestos a procedimientos que implican riesgos para la salud; refieren síntomas no diagnosticados y que podrían afectar el organismo a largo plazo. No se adoptan medidas de prevención y promoción.(AU)


ABSTRACT Objective Determine the Perceived Impact on the Artisanal Miner's Health from Quinchía's Municipality (Colombia) by the Use of Cyanide and Mercury in the Amalgamation Process of Gold. Methods Quantitative, descriptive research in a sample of 28 active miners exposed to cyanide and mercury. The process of gold amalgamation was characterized, a survey of symptom perception was carried out and laboratory tests were carried out to detect the concentration of mercury and cyanide in blood. Results 50% of the miners are in the trade between 21 and 30 years, with an exposure of 2.6 hours/day to mercury and cyanide. 33% of miners have levels of mercury concentration in blood between 4-4.9 micrograms per liter (μg/L), as well as refer to symptoms in the nervous and digestive systems. The results of cyanide in blood are negative. Conclusions Blood mercury concentrations are associated with time in the trade, hours of exposure to mercury and perception of symptoms. The results of cyanide in blood are not coincident with the symptoms reported by the miners. Although the toxicity of these chemical substances is recognized, traditional and prohibited methods are still used without taking into account safety protocols and the use of appropriate protection elements. Miners are constantly exposed to procedures that involve health risks, refer undiagnosed symptoms and that could affect the organism in the long term. Prevention and promotion measures are not adopted.(AU)
